Support for At-Risk Communities
Community Health Services' Immunization & Health Screening Program offers support to community residents to better manage a spectrum of chronic and acute health needs.
Community Health Services (CHS) has provided community flu immunization clinics since 2014. Each year, CHS partners with community members, advocates, minority organizations, senior centers, disability organizations and faith-based institution to offer free community immunization to reach at-risk adults.
In addition to offering flu vaccinations, we also offer free health screening for at-risk communities. Screenings include diabetes, total cholesterol, and blood pressure. Healthcare resources for follow up offered to those clients that screen out of range. For clients without a healthcare provider, that screen out of range, will be provided with a referral to local low cost clinic for future follow-up.
Our program trains U-M nursing, medical and pharmacy students to provide clinical opportunities to deliver efficient and safe vaccinations and health screenings in the community.
